00RESOLUTIONNO.2-05WHEREAS,theTownofEstesPark,Coloradoisauthorizedtoadoptbenefitplans,theTownherebyadoptsthefollowingmeasuresasaformalaction.WHEREAS,theformofCafeteriaPlan,includingaDependentCareAssistanceProgramandHealthCareReimbursementPlaneffectiveJanuary1,2005,presentedtoatthismeetingisherebyapprovedandadoptedandthatthedulyauthorizedagentsoftheTownareherebyauthorizedanddirectedtoexecuteanddelivertotheAdministratorofthePlanoneormorecounterpartsofthePlan;andWHEREAS,theAdministratorshallbeinstructedtotakesuchactionsthataredeemednecessaryandproperinordertoimplementthePlan,andtosetupadequateaccountingandadministrativeprocedurestoprovidebenefitsunderthePlan;andNOW,THEREFORE,BEITRESOLVEDBYTHEBOARDOFTRUSTEESOFTHETOWNOFESTESPARK,COLORADOthatthedulyauthorizedagentsoftheTownshallactassoonaspossibletonotifytheemployeesoftheTownoftheadoptionoftheCafeteriaPlanbydeliveringtoeachemployeeacopyofthesummarydescriptionofthePlanintheformoftheSummaryPlanDescriptionpresentedatthismeeting,andwhichformisherebyapproved.INTRODUCED,READ,ANDPASSEDBYTHEBOARDOFTRUSTEESOFTHETOWNOFESTESPARKonthis//%dayof__________________,2005.TOWNOFESTESPARK,COLORADOATTEST:TownClerk 00
