o0RESOLUTIONNO.27-05WHEREAS,theTownofEstesPark,Coloradoisauthorizedtoadoptbenefitplans,theTownherebyadoptsthefollowingmeasuresasaformalaction.WHEREAS,theformofCafeteriaPlan,includingaDependentCareAssistanceProgramandHealthCareReimbursementPlaneffectiveJanuary1,2006,presentedtoatthismeetingisherebyapprovedandadoptedandthatthedulyauthorizedagentsoftheTownareherebyauthorizedanddirectedtoexecuteanddelivertotheAdministratorofthePlanoneormorecounterpartsofthePlan:andWHEREAS,theAdministratorshallbeinstructedtotakesuchactionsthataredeemednecessaryandproperinordertoimplementthePlan,andtosetupadequateaccountingandadministrativeprocedurestoprovidebenefitsunderthePlan;andNOW,THEREFORE,BEITRESOLVEDBYTHEBOARDOFTRUSTEESOFTHETOWNOFESTESPARK,COLORADOthatthedulyauthorizedagentsoftheTownshallactassoonaspossibletonotifytheemployeesoftheTownoftheadoptionoftheCafeteriaPlanbydeliveringtoeachemployeeacopyofthesummarydescriptionofthePlanintheformoftheSummaryPlanDescriptionpresentedatthismeeting,andwhichformisherebyapproved.INTRODUCED,READ,ANDPASSEDBYTHEBOARDOFTRUSTEESOFTHETOWNOFESTESPARKonthisi-dayofocc..e,2005.TOWNOFESTESPARK,COLORADOtATtEST:ltjwnClerk 00
