CaRESOLUTIONNO.06-08WHEREAS,theTownofEstesPark,Coloradoisauthorizedtoadoptbenefitplans,theTownherebyadoptsthefollowingmeasuresasaformalaction.WHEREAS,theformofCafeteriaPlan,includingaDependentCareAssistanceProgramandHealthCareReimbursementPlaneffectiveJanuary1,2008,presentedtoatthismeetingisherebyapprovedandadoptedandthatthedulyauthorizedagentsoftheTownareherebyauthorizedanddirectedtoexecuteanddelivertotheAdministratorofthePlanoneormorecounterpartsofthePlan;andWHEREAS,theAdministratorshallbeinstructedtotakesuchactionsthataredeemednecessaryandproperinordertoimplementthePlan,andtosetupadequateaccountingandadministrativeprocedurestoprovidebenefitsunderthePlan;andNOW,THEREFORE,BEITRESOLVEDBYTHEBOARDOFTRUSTEESOFTHETOWNOFESTESPARK,COLORADOthatthedulyauthorizedagentsoftheTownshallactassoonaspossibletonotifytheemployeesoftheTownoftheadoptionoftheCafeteriaPlanbydeliveringtoeachemployeeacopyofthesummarydescriptionofthePlanintheformoftheSummaryPlanDescriptionpresentedatthismeeting,andwhichformisherebyapproved.INTRODUCED,READ,ANDPASSEDBYTHEBOARDOFTRUSTEESOFTHETOWNOFESTESPARKonthistIdayof________________,2008.TOWNOFESTESPARK,COLORADO44faorCp1wnClerk 00H
