The services provided during the referenced period were on a part-time/on-call as-needed basis and included observation and field density testing of prepared foundation bearing strata material, foundation wall backfill and of retaining wall backfill material as described herein, observations of foundation bearing strata for acceptance to place/form the cast-in-place concrete foundation system at locations presented herein, observations of foundation reinforcing steel, for various structural related elements, periodic site observations during the concrete masonry unit (CMU) wall construction for the elevator shaft, along with testing of masonry grout and cast-in-place concrete for the referenced project. The results of the site observations and testing services completed by EEC personnel during the referenced period are included with this report. Field Density Testing and Observation Services During the referenced period, EEC personnel performed field density tests on prepared foundation bearing strata material, foundation wall backfill and of retaining wall backfill material as described herein, which was prepared/placed and compacted prior to and/or during our various site visits. The field density tests were completed at random locations and elevations 4396 G REENFIELD D RIVE W INDSOR, C OLORADO80550 (970) 545-3908 FAX (970) 663-0282 www.earth-engineering.com Earth Engineering Consultants, LLC EEC Project No. 1164046 Estes Valley Community Center – Estes Park, Colorado March 22, 2017 Page 2 within the areas tested and were performed in general accordance with ASTM Specification D6938. The field density test results and approximate location of each test completed during the referenced period are shown on the attached summary sheet and the associated site diagrams. The field density test results, in general, conformed to the project specific compaction requirement of at least 95% of standard Proctor maximum dry density (ASTM D698). As requested by the general contractor, EEC personnel conducted visits on February 14 and 21, 2017, and March 3 and 6, 2017, to perform a visual observation of the foundation strata materials’ in-situ conditions prior to placement of formwork and reinforcement of the foundation alignments along approximate gridlines A1-7.5, A2-AB, A5-AC, A6-AE.5, F-8, E-8, D-8, B-8, AB from A.3 to A.5, and of footing pads at B-6, and B-7. The prepared foundation bearing strata as observed on the dates indicated herein appeared suitable for support of the foundation designed using a maximum net allowable bearing pressure of 5,000 psf as per the site-specific geotechnical engineering report. The areas were approved for continuation of the planned foundation construction. Foundation Reinforcement Observations During the referenced period, EEC personnel conducted site visits on February 17, 21, and 23, 2017, and March 1, 2, 6. 7, 8, 9, and 10, 2017, to perform visual observations of the reinforcing steel placed within portions of the building’s foundation alignments, at the gridlines identified on the enclosed “color-coded/highlighted” site diagram, prior to placement of concrete. The reinforcement for the footings/footing pads was placed in general accordance with the “Footing Schedule” as presented on page S300 of the structural drawings. The reinforcement for the various pilasters were placed in general accordance with the “Pilaster Schedule” as presented on page S305 of the structural drawings. The reinforcement for the stem wall and foundation wall alignments were placed in general accordance with the details as presented on pages S301 and S305 “Footing Schedule” as presented on pages S300 of the structural drawings. The vertical dowels continuing into the CMU walls above, where appropriate, were installed to match the CMU reinforcement as per the plans and specifications. The brace frame footing/interior grade beams reinforcement was in general accordance with the details as presented on pages S550 and S551 of the structural drawings. Earth Engineering Consultants, LLC EEC Project No. 1164046 Estes Valley Community Center – Estes Park, Colorado March 22, 2017 Page 3 Any discrepancies of reinforcement during the various daily site visits were brought to the attention of the project’s construction management consultant, and the reinforcement subcontractor, during the initial site visit and corrections/modifications were addressed accordingly prior to placement of concrete. Clearances and lap lengths, where applicable, were checked and appeared to meet the minimum requirements. The horizontal and vertical reinforcement for the portion of the foundation system as noted herein and observed by EEC personnel on the dates indicated, appeared to be in general accordance with details presented on the structural drawings prepared by Martin and Martin Consulting Engineers, dated January 12, 2017, and/or the shop drawings prepared by Barton Supply, dated December 22 and 28, 2016. The shop drawings were used as a reference to provide clarity when specific details were not presented on the structural drawings. The hand written field copies of each daily observation were also provided to the on-site project superintendent for the general contractor. Concrete Masonry Unit (CMU) Wall Construction Observations During the referenced period, EEC personnel observed portions of the reinforcing steel and other structural related elements during the 8-inch by 16-inch standard block CMU wall construction for the elevator core at approximate gridlines A6 between AH and AH.3. Layout of the CMU wall alignments were provided by others. During each daily site visit, (February 23, 24, 27, and 28, 2017, and March 3, and 10, 2017), EEC personnel observed CMU wall construction at the elevator core wall alignments and recorded approximate gridline locations, elevations/courses for each day, along with the observed horizontal and vertical reinforcement requirements and grout placement as needed. The vertical reinforcement for the elevator core, in general, consisted of one (1) #5 rebar spaced 48-inches on-center, along with an additional #5 rebar positioned at each end of walls, at each corner, and along each side of openings, control joints and/or expansion joints. The horizontal reinforcement consisted of 9-gauge “ladder-type” reinforcement spaced every 16-inches on- center full-height of each CMU wall. At the various bond beam locations, below and above windows, beam pockets, and at openings, where noted on the structural drawings, EEC personnel Earth Engineering Consultants, LLC EEC Project No. 1164046 Estes Valley Community Center – Estes Park, Colorado March 22, 2017 Page 4 noted the reinforcement consisted of two (2) continuous #5 within the fully-grouted cell alignments. The horizontal and vertical reinforcement for the portions of the CMU walls and observed by EEC personnel during the referenced timeframe on a periodic basis as noted herein appeared to be in general accordance with details presented on the structural drawings. During the grouting operations, EEC personnel observed grout placement within the low lifts. Lap lengths, where applicable, were checked and appeared to meet the minimum required lengths. During the CMU construction phase for the project, EEC personnel tested/evaluated the grout material used within portions of the wall alignments, as identified herein, (please refer to EEC’s Grout Set No. 1). Grout compressive strength cylinders, (approximately 4-inch diameter by 7- 5/8-inches in length), were molded on-site in general accordance with ASTM specifications. The grout cylinders were molded on-site within CMU blocks used to construct the CMU walls, and were extracted/cored using a 4-inch inside diameter diamond bit core barrel upon retrieval and delivery to our Windsor laboratory 2 days following the initial preparation of the specimens. Results of the field and 7-day laboratory tests performed during the referenced period are provided herein, while the 28-day test results will be provided as the results become available. During the referenced period the project’s masonry subcontractor prepared two (2) sets of concrete masonry unit (CMU) prisms, one of which consisted of three (3) 8-inch singled-celled “hollow” prism and the second of which consisted of three (3) 8-inch singled-celled fully grouted CMU block prisms, for compressive strength testing evaluation. Results of the 7-day laboratory compressive tests performed during the referenced period for the prisms are included herein, while the 28-day fully grouted CMU prism test results will be provided as the results become available. Testing of Cast-in-Place Concrete During the referenced period, EEC personnel observed and tested Portland cement concrete delivered to the site for placement as indicated on the enclosed daily reports for the referenced project, (please refer to Set Nos. 12 through 22). Field testing performed on the plastic concrete included slump, air content, unit weight, and concrete temperature.
