4/7/09- Per AC: left a message for the Beerys and said that we needed a site plan.
I suggested that they take a look at the current site plan we have on file. That
provided they verify that the house is in the location shown on the plan. That thEy
could just get a copy of this site plan and show the new addition on it. From takiig
a look at the site plan and doing a site visit yesterday, I think it will comply with
setbacks. Just cannot assume the old site plan is accurate without some
verification from the property owner.

PROJECT MANAGEMENT & ALL TRADES / SU CONTRACTORS
1. All construction shall comply with applicable codes as amended and adopted by the Town of Estes Park.
2. Issues/details on the approved plans that exceed minimum code requirements shall be enforced.
3. All staff review comments are requirements of permits and are included in the permit packet,
4. The permit applicant shall maintain the entire permit packet available on the job site. The permit packet is to be
located at the posted address. The permit packet is to be kept inside a new/used electrical panel box,
painted red with the address # on the top front & the word "permit" on the bottom front (minimum
external dimension: 14" x 19" x 5"). Temporary addressing for new construction is to be located at the
drive entrance along with the permit package. Failure to comply with the addressing requirements will
result in delayed inspections and re -inspection fees.
5. Construction shall comply with approved submittals. (Anchor bolts, hold-downs, shear wall details, number and
size of trimmers and king studs, floor and roof diaphragms, truss blocking and bracing, roofing materials, fastening
schedule, specified connectors and fasteners, etc.)
6. All work shall be performed and completed in strict compliance with the approved plans. The designer of
record and the Department Plans Examiner or Building Official shall approve any and all changes or
revisions,
7. Plans have been reviewed and approved with comments. Follow the Town approved plans or provide
revisions. Major changes shall incur additional review fees of $100.00 per hour, and result in project time
delays,
8. Other departments ay have requirements that need to be addressed before ssuanceissuance of a CO.
9. Approved plans are not for field construction use. They are for the exclusive use of the Town inspectors. If
approved plans are not available or readable, inspections may not be provided and re -inspection fees may be
assessed.
10. If Town staff has to reproduce an approved set of plans, fees shall be assessed.
11. Pre approved design build items (such as plumbing, HVAC systems) are subject to approval during field
inspections, and shall comply with all locally adopted codes as amended.
12. Each contractor shall obtain a business license from the Estes Park Town Clerk prior to beginning
work, per EPMC, Chapter 5.20.040
13. An approved contractor affidavit shall be submitted to the Town Clerk prior to requesting final inspections
and signed off by the Town Clerk and Chief Building Official. A final inspection will not be done until the
contractor affidavit has been submitted to the Building Department, If contractors do not have
business licenses, the TCO/CO will be held until such time as licenses are procured. See Contractor
Affidavit handout and Trades Checklist provided with this checklist,
14. The inspection card is required to be on site for all inspections. A CO can not be issued without all required
inspections signed off on the inspection card.
15. Living in construction trailers and/or recreational vehicles is prohibited.

project, This includes mitigation of soil erosion/dust pollution created by water and wind. it also includes

17. Toilet facilities shall be provided for construction workers. 2003 IPC 311.1
18. Nothing shall be concealed before inspection and approval.
ENERGY
19. Sources of air leakage (infiltration and exfiltration) are to be sealed to limit uncontrolled air movement per
IRC N1102.1.10. i.e.: weather stripping attic accesses, caulking exterior wall penetrations, etc.
STRUCTURAL
20. Notching of LVL's require engineering. IRC 502.8.2.
21. Construction shall be adequately braced to resist all loads, including wind, This includes temporary bracing.
22. Provide squash blocks at all concentrated load points, per IRC.
23. Proprietary engineered components shall comply with the more restrictive requirements either from the
manufacturer or engineer of record.
24. Field alterations/modifications to pre-engineered components require job specific and component specific
engineering, by the Truss Manufacturer and/or Structural Engineer.
25. All joists, rafters, and trusses shall be laterally blocked at bearing points to prevent rotation, IRC 502.7
(Admin. Policy — wind)
26. All roof framing members shall be connected to resist uplift. Truss clips shall be listed for their use and
installed per their listing.
27. Roof over -frames require minimum 2 x sleepers.
28. Roof look outs shall be connected to resist uplift at exterior bearing wall. Start at lowest point and secure at 48"
o.c. maximum up the rake.
29. All wall bracing shall be installed per the approved plans and require inspection,
30. All glazing shall be installed prior to Rough Frame Inspection.
31. All hold-downs, straps, etc. shall be inspected prior to concealing.
32. All welders shall provide a copy of AWS Certification.
33. All fasteners into ACQ treated wood must be of an approved type.
34. Framing inspections shall not be provided or approved prior to all other rough inspections.
ELECTRICAL
35. All electrical contractors and their employees shall comply with licensing and permitting requirements of
Colorado Revised Statutes (CRS) Title 12, Article 23, and shall carry their pocket cards with them on the
job site.

Dear Carolyn,
1 have analyzed the proposed changes to this structure regarding the new roof system/dormers. While the
loads will barely increase (same upper level .floor loads and same roof loads) the dead loads will increase a
little bit with the weight of the new upper level walls for the dormers. IX.1e to this load increase 1 have
looked at the headers/beams which are below the areas of the new loads. I have found that the header over
the master bedroom west window (west side of bearing for the root) is currently sufficient and will handle
these new dead loads as well. This 4'-8" hears is made up of 2, 6"ta11 and 6" wide eastern white pine logs
(thus a 6xI2 member).
The I 2'-4" long beam (1, 6x6 eastern white pine) which supports the eastern hearing for the roof and upper
floor system is currently undersized; thus 1 do not recommend increasing the load on this beam without
some changes.
This beam has an available new bearing location for a column at 7'-6" from the end. I suggest a column be
put in this location. 1 have verified that there is an existing spread tooter below this column location, thus
no foundation work will be required. Squash blocks, or a continuous column to the limier, will be required.
Additionally, this beam will still need reinforcing. I have found a 5.5" tall, 2,0 LVL, screwed to one side of
this existing beam will be needed. The screws should he 6" long 'limber Loks. 1The screws should be
spaced 6" 0.C,, staggered. while holding an 1-1/2" edge distance. At each bearing point there should be
_4 .1
screws, again holding an1-1/2" edge distance,
