All construction shall comply with applicable codes as amended and adopted by the Town of Estes Park. 2. Proprietary engineered components shall comply with the more restrictive requirements, of either from the manufacturer or approved plans. 3. All work shall be performed and completed in strict compliance with the approved plans. The designer of record and the Department Plans Examiner or Building Official shall approve any and all changes or revisions. 7. Plans have been reviewed and approved with comments. Follow the Town approved plans or provide revisions. Major changes shall incur additional review fees of $100.00 per hour, and result in project time delays. Other departments may have requirements that need to be addressed before issuance of a CO, Approved plans are not for field construction use. They are for the exclusive use of the Town inspectors. Each contractor shall obtain a business license from the Estes Park Town Clerk prior to beginning work, per EPMC, Chapter 5.20.040 13. An approved contractor affidavit shall be submitted to the Town Clerk prior to requesting final inspections and signed off by the Town Clerk and Chief Building Official. A final inspection will not be done until the contractor affidavit has been submitted to the Building Department. If contractors do not have business licenses, the TCO/CO will be held until such time as licenses are procured. See Contractor Affidavit handout and Trades Checklist provided with this checklist. 14. The inspection card is required to be on site for all inspections. A CO can not be issued without all required inspections signed off on the inspection card. 15. Temporary construction office trailers require permits. 16. Living in construction trailers and/or recreational vehicles is prohibited. 17. Temporary signs may require permits. Renovation of existing areas that include a change of use or expose non compliant conditions are required to be brought into compliance. 21. Nothing shall be concealed before inspection and approval. HAZARDOUS MATERIALS 22. Advise the Department of Building Safety and other appropriate authorities of all Hazardous construction materials on site (i.e. asbestos, radiation, compressed gasses). SITE WORK (EXCAVATION - GRADING - PAVING) 23. Call before you dig. See Call Before You Dig handout. 24. No activity in any public right-of-way is approved by building permits. Right-of-way permits are required by the Town Public Works Department. (Greg Sievers 970-577- 3586) and by CDOT (Gloria Hice-Idler, Access Coordinator 970-350-2148). 25, Floodplain Development Permits are required for any and all construction activity within the floodplain. The limits of site disturbance shall be properly maintained. 29. Erosion control shall be properly maintained. (Silt fences, straw bales, rip rap, etc.) 30. A properly installed silt fence shall continue uninterrupted, per the approved plans. 31. All storm water shall be properly filtered prior to leaving the site. 32. Obtain and submit copies of all required Federal and State Storm Water & Waste Water discharge permits. (Colorado Dept. of Health, Water Quality Control Division, 303-692-3500). 33. Site housekeeping (litter, dust, materials, etc.) shall be properly maintained. This includes mitigation of soil erosion/dust pollution created by water and wind. It also includes mud and debris in the Public right-of-ways. 34. Erosion, whether by wind or water, shall be controlled and maintained by the proper use of silt fences, straw bales, rip rap or other approved means. Storm water control and sediment control shall be the owner/builders' responsibility and shall not affect adjacent properties, 35. Stock piling of soil shall comply with property line and Right of Way requirements, per IBC, J108. See Stock Piling of Soil handout. FOUNDATION 36. Provide copies of all compaction and other geo-tech tests and reports. 37. A Setback certificate sealed and signed by a Colorado registered land surveyor is required prior to placing foundation walls. Provide original stamped letter in permit package. Information provided will include actual measured distances. 38. All foundation anchor bolts shall be installed per project engineering and 2003 IBC, Table 1912.2. 39. Timed concrete inspections may be requested, and shall be provided within a two-hour window (+/- one hour). Rebar must be in place 100%. Grade must be established on forms 100%. Wall forms need not be installed 100% for inspection. 40. All foundations require subsoil drainage systems, per IBC, 1807.4.2; and are considered part of the foundation system design. Subsoil drain water shall not discharge into the public sewer system. Inspections of the waterproofing/damp proofing and subsoil drain are required. 41. Provide positive drainage away from all buildings, 42. Do not backfill against foundations and retaining walls until all supporting slabs and/or floor framing are in place and securely anchored or until adequate temporary shoring is installed. STRUCTURAL 43. Construction shall be adequately braced to resist all loads, including wind. This includes temporary bracing. Truss clips shall be listed for their use and installed per their listing. 51. All shear walls shall be installed per the approved plans and require inspection. 54. All hold-downs, straps, etc. shall be inspected prior to concealing. 55. Fire blocking is required in concealed spaces in combustible construction. See Fire Blocking handout. 56. All welders shall provide a copy of AWS Certification. 57. Framing inspections shall not be provided or approved prior to all other rough inspections. ELECTRICAL 58. All electrical contractors and their employees shall comply with licensing and permitting requirements of Colorado Revised Statutes (CRS) Title 12, Article 23, and shall carry their pocket cards with them on the job site. 59. Electrical permits and inspections are the responsibility of the Colorado State Electrical Board, Department of Regulatory Agencies. (Dave Cearlock 970-577-3589). 60. The State Inspector approving the electrical installations shall sign the Department of Building Safety Inspection Card. 61. General Condition 11 is amended by adding the following condition for work in perennial or intermittent waters of the United States: If heavy equipment is used for the subject project that was previously working in another stream, river, lake, pond or wetland within 10 days of initiating work, one of the following procedures is necessary to prevent the spread of New Zealand Mud Snails and other aquatic hitchhikersi (1) Remove all mud and debris from equipment (tracks, turrets, buckets, drags, teeth, etc.) and ceep the equipment dry for 10 days; or (2) Remove all mud and debris from equipment (tracks, turrets, buckets, drags, teeth, etc.) and spray/soak equipment with either a 1:1 solution of Formula 409 Household Cleaner and water, or a solution of Sparquat 256 (5 ounces Sparquat per gallon of water). Treated equipment must be kept moist for at least 10 minutes; or (3) Remove all mud and debris from equipment (tracks, turrets, buckets, drags, teeth, etc.) and spray/soak equipment with water greater than 120 degrees F for at least 10 minutes. Gengler, Hattie Schetzsle, Rick O'Dell Guests: Rick Robbins, R.R.I, Inc., Marie Richardson Prior notice of this meeting was given by posting a notice at the Town of Estes Park's Municipal Building, the Estes Park Public Library, Estes Valley Recreation and Park District's Administration Office, the Larimer County Clerk's Office in Estes Park, and advertisements in the local newspapers and media. Kathy Asche called the meeting to order at 5:07 P.M. There were two agenda items: The Lake Estes Golf Course Irrigation System Replacement Bid Award, and the Reciprocity Agreement between the Recreation District and the Park School District R 3. Lake Estes Golf Course Irrigation System Replacement: The District received five bids for the Lake Estes Golf Course Irrigation System Replacement. The companies were all from out-of-state. The bids submitted are: • American Landscape, Inc., Canoga Park, CA. — $494,692.15 • Aspen Corporation, Charlotte, N.C. — $476,791.92 • Fonnost Construction, Temecula. CA. — $581,789 • Landscapes Unlimited LLC, Lincoln, N.E. — $504,900 • Mid America Golf and Landscape, Lee's Summit, MO. — $665,000 Bids for the two Alternates (Alternative 1=Water line; Alternative 2=Drainage) are: • American Landscape, Inc. — Alternative 1: $1,380; Alternative 2: $2,100 • Aspen Corporation — Alternative 1: $3,975.91; Alternative 2: $3,898.13 • Formost Construction — Alternative I: $3,000; Alternative 2: $12,836 • Landscapes Unlimited LLC — Alternative 1: $2,900; Alternative 2: $4,400 • Mid America Golf and Landscape — Alternative 1: $5,000; Alternative 2: $4,000 Rick Robbins, from R.R.I. Inc., and the Architect on the Irrigation project, discussed the requirements of the bid, the particulars of the bids received, and the company references. He pointed out that the bids include a "rock clause" which lists the costs associated with removing underground rock during construction. Rick pointed out that this is an unknown, albeit anticipated, aspect of the project. The rock clause estimates from bidding BRADFORD PUBLISHING CO. RECORD OF PROCEEDINGS companies varied from $2/linear foot to $45/linear foot. Depending upon the amount of rock encountered, the cost for change orders could be quite large. Rick also passed around examples of HDPE pipe, explained how it is manufactured, and the advantages of using it on mountain golf courses. Given the credentials and mountain golf course experience, as well as the other factors including references and the rock clause fee, Rick recommended that the Board award the contract to Landscapes Unlimited, LLC from Nebraska Moved by Jack Holmquist, seconded by Ben Greer, to award the bid to Landscapes Unlimited, LLC for the Lake Estes Golf Course Irrigation System Replacement project in the amount of $504,900, with Alternative #1 not to exceed $2,900 and Alternative #2 not to exceed $4,000. No further discussion. Ayes — 5. Motion carried unanimously. The Board next reviewed Resolution 10-7 authorizing the District to enter into lease purchase agreements. The Bank of Colorado is the lease purchase financial mechanism to fund the golf irrigation system at the Lake Estes Golf Course. The District is providing 77 acre feet of water as collateral for the lease purchase of the irrigation system. Moved by Jack Holmquist, seconded by Ben Greer, to approve Resolution 10-7, which states that the District has the authority to enter into lease transactions to provide necessary equipment or other property used, or to be used, for District purposes, being the Lake Estes Golf Course Irrigation System Replacement, and authorizing the President and Secretary of the Board, or other members of the Board, to execute and deliver on behalf of the District such certificates, tax information, returns and other documents as may be necessary or desirable to complete the lease transaction. No further discussion. Ayes — 5. Motion carried unanimously. The Board then discussed whether or not to release an RFP for the pump house portion of the irrigation project. The consensus of the Board is to bid the pump house, unless time becomes a constraint, in which case the Board approves the price submitted by Mountain Concrete for the pump house. Reciprocity Agreement: The Board received a revised Reciprocity Agreement between the Recreation District and the Park School District R-3 on the day of the meeting via email from Todd Jirsa, President of the Estes Valley School Board. Changes include a new paragraph in Section 2 of the agreement stating: Both the Rec District and Park R-3, believing that young people benefit greatly from participation in healthy and age appropriate activity, agree to encourage the community's youth to participate in Rec District and Park R-3 activities. Further, Park R-3 and the Rec District agree that Stanley Park being a public facility, should be made available for recreational use by the community's youth whenever feasible. Since the Estes Valley School Board has not approved or signed the revised Reciprocity Agreement, EVRPD's Board will take no action on the revised agreement sent to them from Todd Jima. Discussion: Marie Richardson, a member of the Estes Valley School Board, was in attendance and asked which Board approves the document first, given that it is an Intergovernmental Agreement. The District Board was unsure of the answer to this question. B RAOFORD PUBLISHING CO. RECORD OF PROCEEDINGS Discussion: The revised agreement and additional wording duplicates the spirit of the original agreement, the wording is too narrow, yet open to interpretation. Moved by Jack Holmquist, seconded by Mike Richardson, to approve the Reciprocity Agreement between the Recreation District and the Park School District R 3 as originally presented to the Recreation District.
