Town Administrator SUBJECT: Administrative Committee Agenda Discussion WORKER'S COMPENSATION - DESIGNATION OF MEDICAL PROVIDER The Worker's Compensation law allows us, as an employer, to choose the treating physician(s) for our employees' work -related injuries and illnesses. Andy Wills and his assistant, Teri Egan, will be present to present a proposal from Estes Park Medical Center to be designated as the Town's medical provider for Worker's Compensation services. Two attachments are included explaining the proposal in more detail. There is no cost to us, and the potential is there for savings on our insurance premiums. Recommendation: Recommend the Town designate Estes Park Medical Center as our medical provider for Worker's Compensation services. LIFE INSURANCE BIDS Bids have been taken for the life insurance benefit added for 1992. A bid summary and memo from Monte Vavra/Finance Officer is included in the packet. Monte will present the results of the bids. Recommendation: Recommend the Town Board accept the bid of Group America Insurance Company. P. O. Gary Klaphake/Town Administrator will explain the proposed changes. Recommendation: Recommend approval to the Town Board of the amended organizational chart. XEROX MACHINE LEASE/PURCHASE AGREEMENT Attached is a memorandum from Vickie O'Connor/Town Clerk explaining this item. Recommendation: Recommend approval to the Town Board of the xerox machine lease/purchase agreement. RESOLUTION TO CARRY FORWARD ENCUMBERED FUNDS FROM 1991 TO 1992 Our encumbrance system which began in 1991 allows us to "carry over" funds for contracts and other commitments from one budget year to another without budgeting money for the same expenditure in both years. An example of this is the Marys Lake Water Plant. To accomplish this routine housekeeping measure, a resolution (enclosed in your packet) is required. Monte Vavra/Finance Officer will explain in more detail if needed. Recommendation: Recommend to the Town Board, approval of the resolution to reappropriate 1991 encumbered funds to the 1992 budget. Inc. Occupational Health Services Estes Park Medical Center is introducing occupational health services designedto assist you with the treatment and management of work -related injuries. We can provide quality care to injured workers so they may return to work just as soon as they are physically able; and provide personal communication with our affiliated employers so they stay informed of the medical status of their employee and when they can return to work. It's a program designed to "work" for both the employee and the employer right here in Estes Park. Through Estes Park Medical Center's occupational health services... Your employees get... competent medical care any time day or night. An injured employee has 24 hour access to medical care and is treated as soon as possible. Even specialty care is available locally through our Specialty Clinic. Affiliated employers get... case management for their injured workers. This means they have a single contact person who will provide reports on the employee's progress, explain the plan of treatment and handle all the paperwork. The Nurse Case Manager will work on behalf of the injured worker and the employer to see that all necessary treatment is provided for a successful recovery in a cost effective manner. This is how it works... when there's an on-the-job illness or injury. The injured worker comes to the admission office adjacent to our emergency department and registers. Our registration procedure will identify them as an employee of an Affiliated Employer. First, the employee will be evaluated and treated by a physician in our emergency department. Once the injury has been assessed, the physician will complete aWotrlk Status o Report detailing the nature of the injury, the employee's work status, and any on-the-job limitations or restrictions. Arrangements will be made for any follow-up visits or specialist referrals. This information is discussed with the employee, who is given a copy of the report to return to his employer, and one to retain for his files. Soon after your employee has been treated, your company will receive a work status phone call from our Nurse Case Manager who will discuss the specifics of the case, the plan for treatment and answer any question you might have. All follow-up appointments will be scheduled and provided at Estes Park Medical Center. If a specialist referral is indicated, our Nurse Case Manager will continue to follow the case and maintain the lines of communcation between your company, the employee, and the insurance company. Conclusion... You want to be sure your employees receive prompt, effective treatment for everything from cuts and sprains to severe on-the-job emergencies while also controlling your workers compensation expenses. Vavra, Finance Officer DATE: January 27, 1992 SUBJECT: Life, Accidental Death and Dismemberment Insurance Bids The Town recently requested bids for life insurance at one times the employee's annual salary, up to a $50,000 maximum, and accidental death and dismemberment insurance. The Town wanted the policy to cover all active employees, excluding elected officials, without any rating based on medical history. I have received bids back from six insurance agents representing seven different companies. The apparent low bid is Companion Life Insurance at $.29 per $1,000 of payroll, represented by Denton Insurance Agency. The company has an A rating. Floyd Denton, as agent, has recommended that the Town consider the second low bid of $.30 per $1,000 of payroll from Group America Insurance Company. This company is rated A+, has a history of offering better service, and has an additional benefit if a covered employee is killed in an automobile accident and is wearing a seat belt. It also requires no medical history as long as employees are actively at work on the effective date. I concur with Mr. Denton that the Administrative Committee should recommend to the Town Board to proceed with the purchase of the Group America Insurance plan as bid. This would cost $9,252 per year using an annual payroll of $2,570,000. Sufficient funds were budgeted in 1992 to cover this cost. Estimated effective date would be February 1, 1992, or as soon as possible thereafter. Due to the volume and quality of copies, $8,960 was included in the 1992 Budget for the purchase of a new machine. Xerox Corporation placed a demonstration unit in the office in December for a trial period. This machine includes an automatic document feeder, 10-bin sorter, high capacity feeder, stapler and automatic duplexing and reduction features. During the 1992 budget process, staff considered a lease vs. an outright purchase. Due to budget constraints, the lease option was selected as we would receive a 48-month warranty with no maintenance fees; monthly payments are $185.00. The Administration Department requests authorization to trade-in our existing Model 1038 and lease the Xerox Model 5034. RESOLUTION A RESOLUTION TO REAPPROPRIATE THE TOWN OF PARK'S 1991 ENCUMBERED FUNDS TO THE 1992 BUDGET. WHEREAS, the Town of Estes Parks accounting system incorporates a purchase order system wh,iC h encumbers the budget when commitments are made; and /f WHEREAS, encumbrances that were not liquidated in calendar year 1991 are reserved from the Ending Fund Balances; and WHEREAS, the Town of Estes park intends to carry forward to 1992 the encumbered budget authgfity. NOW, THEREFORE, BE IT RESOLVED BY THE BOARD OF TRUSTEES OF THE TOWN OF ESTES PARK, COLORADO, THAT: 1.
