GAS VENTING INSTALLATION
AFFIDAVIT OF COMPLIANCE

The following conditions must be met before submitting an Affidavit:
x The contractor shall be pre -approved by the Chief Building Official
The work shall be authorized by the appropriate permit
The work shall be associated with an approved system
The affidavit shall be on a form provided by the CBO

The Division has pre -approved the gas flue venting installation to be completed without Division
inspection.
The installing contractor shall complete and sign this affidavit.
The affidavit shall be left in the permit pack at the jobsite.
The work and conditions described on the permit I approved plan matches the field installation.
Pre-existing venting is compatible With the new appliance.
Venting within concealed spaces is properly sealed, sloped and supported.
Venting within combustible wall or chase has the required clearances to combustibles.
Masonry chimney relined with venting has been cleaned and structurally repaired as necessary.
An approved section of vent has been used to get around the smoke shelf.
An approved draft hood connector or ether approved device has been used.
The damper has been fixed as per installation instructions.
The termination cap has been sealed and secured as per installation instructions.
Existing venting connected to the new venting is not corroded from continuous conden,,sation;.
(OPTIONAL) Photos of the ongoing installation have been provided.
With my signature I verify the gas flue venting installation was performed in accordance with the manufacturers'
Installation specifications, Chapter 24 of the 2009 IRC or Chapter 5 of the 2009 IFGC and I release the Town of
Estes Park from all ability*.
