contractor failed to obtain required mid roof inspection as required by sec-. 109.1.5.
1. The description of work and conditions described on the permit matches the field installation.
2. The roof is not ithin a wildfire hazard area.
3. Shingles have been fastened per manufacturer instruction (on packages) for high wind area.
4. The underlayment was found to be in an acceptable condition or has been replaced.
5. The flashing was found to be in an acceptable condition or was replaced.
6. All roof areas less than 4 12 pitch have been fully protected by an ice water shield.
7. Fasteners are a minimum No. 12 gage, 3/8"-diameter-head galvanized or other approved nail,
8. Fastener penetration is 3/4" into or through the sheathing, whichever is less.
9. There are no fasteners within 6" of the centerline of the valley.
10. All exposed fasteners have been sealed for weather protection.
11. Ventilation has been provided; minimum 1 sq. ft. per 150 sq. ft. attic space.
12. A final inspection request has been called in.
13. The permit and inspection record card are on site and accessible.
14. A ladder has been provided for access to inspect all areas of the new roof.
