C0TOWNOFESTESPARKC.I.R.S.A.LossControlStandardsFiscalYear1992TheTownofEstesParkhashadanexcellentclaimlosshistorysincejoiningC.I.R.S.A.in1985.TomeetthelosscontrolstandardestablishedforDecember31,1992,thatrequireannualgoalsandobjectivestocontrolthefrequencyandseverityoflossesortheaccomplishmentofspecificsafetyprograms,theTownofEstesParksetsthefollowinggoalsfor1992:i.completeandadopttheC.I.R.S.A.recommendedlosscontrolstandardsthatapply.2.Establishanaccidentinvestigationcommitteetorevieweachaccident/claim.3.Establishaperiodicsafetyinspectionprogramofbuilding,grounds,equipment,machinery,workpractices,andconditions.Acopyofeachinspectionwillberetainedbydepartmentsupervisor.4.Establishasystemtodocumentalltrainingthateachemployeehasreceived.Anannualrecapcopyofeachemployee’strainingwillbekeptinemployee’spersonnelfile.5.Establishandpostanemergencyplanforeachlocation.6.Conductadefensivedrivingseminarforallemployeeswhodriveatownvehicle.7.Establishasystemforannualmotorvehiclerecordreviewofallemployees.8.Establishaninspectionprogramforstreetsigns,roads,andbridges.9.Establishablastingpolicy.10.Establishapolicyforlocatingundergroundutilities.11.Establishapolicyfortrenchingandexcavatingundergroundutilities.12.Establishapolicyforconfinedspaceentrysafety. CCTOWNOFESTESPISRKSafetyPerformanceEvaluationAllemployeesarereviewedfortheirsafetyperformanceduringtheirannualreviewprocess. 0CTOWNOFESTESflflLossControlRecommendationsTheTownofEstesParkwillrespondtoC.I.R.S.A.’slosscontrolrecommendationswithinsixty(60)days.CorrectiveactionwillbeimplementedimmediatelyifrecommendationsareacceptedbytheTownandarewithinthecurrentyear’sbudget.Recommendationsthatwillrequiresignificantexpenseswillbereferredtothefollowingyear’sbudgetcycleandthencorrectiveactionwillbedetermined,unlessanemergencyisdeemedtoexist.Thenimmediateactionmaybetaken. CCTOWNOFESTESPARKACCIDENTINVESTIGATIONPOLICYThePurposeofAccidentInvestigation:Millionsofwordshavebeenwrittenexplainingtheimportanceandpurposeofaccidentinvestigation,butforthefront—linesupervisortheycanallbesummarizedinoneword—PREVENTION.Anaccidentis“anundesiredeventthatresultsinpersoninjuryand/orpropertydamage.”Beingundesiredmakesitsomethingthatmustbepreventedwheneverpossible.Thepurposeofaccidentinvestigation,then,isaccidentprevention.Careshouldbetakentodistinguishbetweenpurposeandbenefits.Benefitiswhatasupervisorgetsfrominvestigatingaccidents,butthepurposeiswhyhedoesit.Therearemanybenefits,whichwillbegivenlater,butonlyonepurpose-PREVENTION.WhyAccidentsMustbeInvestigated:Investigatingaccidentsisaresponsibilityofalllevelsofmanagementandaconcernofeveryemployee,butthelinesupervisors’uniquepositiongivesthemspecialpriorityandresponsibilityinthisfunction.Firstlinesupervisorsintheareawhereanaccidentoccurshavecertainqualificationsandadvantagesothermembersofmanagementdonothave.Frontlinesupervisors:1.knowthemostaboutthesituation.Theyhavedailycontactandfamiliaritywiththeemployees,machines,andmaterialsinvolved.Theyknowthestandardpracticesandcircumstancesinthearea,aswellasthehazards.2.haveapersonalinterestinidentifyingaccidentcauses.Togoodsupervisors,thesearenotsimplyaccidentfiguresandstatistics;butaretheiremployees,theirmachines,andtheirmaterials.Supervisorshaveapersonalinterestintheirprotection.Accidentinvestigationfocusesawelcomelightontheconditionsandhazardsthatcouldendangerthelivesofemployeesordamagedequipmentandmaterial.3.cantakethemostimmediateactiontopreventanaccidentfromrecurring.Beingindirectcontrolofthepeople,procedures,andpropertyintheareagivessupervisorstheadvantageoftakingimmediatecorrectiveactionandthegreatestopportunityforeffectivefollow—up. .c.CAccidentInvestigationPage24.cancommunicatemoreeffectivelywiththeemployees.Employeesmaybe“employedby”theTown,butthey“workfor”thelinesupervisors.Theyknowsupervisorsareinterestedintheirsafety.Tothem,firstlinesupervisorsarethemanagement.Supervisorsspeaktheirlanguage,andevenmoreimportant,understandit.Inaccidentreportingandinvestigation,employeescantellitlikeitistofront-linesupervisors.HowAccidentInvestigationBenefitsSupervisors:Asmentionedearlier,therearemanywayssupervisorsbenefitfrompromptandthoroughaccidentinvestigation.Thefollowinglistisnotmeanttobecompletebutdoesincludesomeofthemajorbenefits.1.Promptandthoroughinvestigationisconcreteevidenceofaconcernforyouremployees.Assupervisors,youknowhowimportantthetrustandrespectofyouremployeesisingettingthejobdone.Employeesnotonlyexpect,butneed,supervisorswholookoutfortheirbestinterests.2.Effectiveaccidentinvestigationincreasesproductionbyminimizingtimelostduetorecurringaccidents.Thistiesindirectlywithreductionofcosts,sinceaccidentsarenotonlytimeconsumingbutexpensive.3.Sinceaccidentinvestigationandresultingpreventionaretangibleevidenceofsupervisors’abilitiesandefficiencies,theyareincreasinglybecominganintegralpartofperformanceevaluation.Agoodsafetyrecordspeakswellofsupervisors’capabilitiesandhassignificantinfluenceontheirprofessionalpossibilities.4.conscientiousinvestigationandmeaningfulcorrectivemeasuresarethemarkofcapablesupervisors;oneswhoareincontrol.Itistremendouslyimportantthatemployeesfeeltheirsupervisorscanhandlethings.Theytakeprideinworkingforsomeonewhocandothejob. 00AccidentInvestigationPage3TheCriticalImportanceofAccidentReporting:Nomatterhowconscientiousfirstlinesupervisorsmightbe,theycannotinvestigateanaccidentuntiltheyareawareofit.Foraccidentinvestigationtohaveaneffect,itmustbeprecededbytotalaccidentreporting.Certainly,majorpropertydamageandseriouspersonalinjuryareseldomaproblemfromthestandpointofreporting;butwhatabouttheminorinjuryorpropertydamage,theapparentlyunimportantlittleaccident?Itisinthisunspectacularareathatfailuretoreportusuallyoccurs.Afterall,whybothersupervisorswithalittlecutonthefingeroronelittlefrayonacable?Becausetheyareaccidents,that’swhy;andsupervisorsmustbeinterestedinaccidentswithoutqualificationastosizeordegree.Whoknowsifanaccidentisminorormajorunlessithasbeeninvestigatedproperly?A.TheFallacyofUnimportantAccidents:Thereisnosuchthingasanunimportantaccident.Theimmediateresultsoreffectoftheaccidentmaybeclassifiedasminor,serious,ormajor,butthisinnowaymeanstheaccidentitselfisunimportant.Thecablethatsuddenlybreaksprobablystartedwithasinglewirethatfrayed.Theinfectionthatrequiredpossibleamputationmaywellhavestartedwiththesmallestcutorscratch.Beyondtheseobviousdangersaremanyothers.Theunreportedaccidentcannotbeinvestigatedoritscausecorrected.Itbecomesatimebombwaitingtobetriggeredagain.Thefactthattheresultsoftheaccidentwereminormaybepure,blindluckorchance.Thenexttinetheoddsmaybedifferent,andtheresultcouldbemajorpropertydamage,seriousinjury,orevenafatality.Foreveryaccidenttobeinvestigated,everyaccidentmustbereported.B.WhyMinorAccidentsmaynotbeReported:Therearemorereasonsforpeoplefailingtoreportaccidentsthancouldpossiblybegivenhere,butsomeofthemorefrequentlyencounteredare:1.notwantingtospoilthesafetyrecord.Failuretoreportminoraccidentsisthequickestwaytodojustthat.2.notwantingtogotoaphysicianfortreatment.Wouldemployeesrathergotothehospitalforextendedtreatment? 00AccidentInvestigationPage43.beingafraidof“kidding”byotheremployees.Thereisnothingfunnyaboutmajordamageorseriousinjury,whichcouldeasilyhappennexttime.Whereisthehumorinaseriousinfection?4.beingafraidofmedicaltreatment,Cleaningasmallwoundisnotnearlyaspainfulasmajorsurgeryforinfection.5.notlikingtheredtapeinvolvedwithalltheformsatthephysician’soffice.Ahospitaladmissionordeathcertificateismuchmorecomplicated.6.beingafraidtheforemanwillgetmad.Agoodforemanonlygetsmadwhenaccidentsarenotreported.Whentheyarereported,hegetsinterested.7.notwantingtobethecentralpersoninanaccidentinvestigation.Thisisreallylikesayingyoudonotwanttobepartofpreventingmajordamageorpersonalinjury.8.notwantingtolosetimefromthejob.Reportingaccidentsisthebestwayofinsuringminimallossoftimeoffthejob.C.PositiveStepstoInsureReportingofAllAccidents:Thefirstlinesupervisorshavetheinfluenceonaccidentreporting.Thereareseveralthingstheycandotoinsureminoraccidentswillbereported.Fourofthemostimportantare:1.trainingnewemployeestoreportallaccidents,nomatterhowsmall,aswellasthe“nearmiss”incidentswhereonlyluckdeterminedtheoutcome.Makesureemployeesunderstandwhythisissoimportant.Explainthatsituationscannotbecorrectedunlesstheyarereported,andanaccidentcouldrepeatitselfwithfarmoreseriousresults.2.encourageemployeestoreportallaccidentsandnearmisses.Youhavetoldthemitisimportant,sotreatthemasimportant.Letemployeesknowyouappreciateitwhentheyreportanaccident.Theywillformtheirideaofhowimportantaccidentreportingisfromthewayyoutreatit.Anequalpartofthisactionistomakeitquiteapparentthatyouaredissatisfiedwithlatereporting.Donot“chewhimout,”butmakeitclearaccidentsmustbereportedimmediatelyandwhy. CCAccidentInvestigationPage5Beforegettingintothe“how”ofinvestigatingaccidents,thereareafewpointsthatweshouldfullyunderstand.TheWhat,When,andWhyofAccidentInvestigation:A.WhatisanAccidentInvestigation?Anaccidentinvestigationisbasicallythesupervisors’analysisandaccountofanaccidentbasedonfactualinformationgatheredbyathoroughandconscientiousexaminationofallfactorsinvolved.Itisnotamererepetitionoftheemployees’explanationsoftheiraccident.Trueaccidentinvestigationincludestheobjectiveevaluationofallfacts,opinions,statements,andrelatedinformation,aswellasdefiniteactionstepstobetakentopreventarecurrence.B.WhenistheTimeforAccidentInvestigation?Thetimeforaccidentinvestigationisalwaysassoonaspossible.Thelesstimebetweenaccidentandinvestigation,thebettertheinformationwhichcanbeobtained.Factsareclearer,moredetailsremembered,andtheconditionsarethoseatthetimeoftheaccident.Theonlysituationswhichshouldbepermittedtodelaytheinvestigationarewhenmedicaltreatmentisneededortheemployeeisemotionallyupset.Naturally,thesupervisorthinksoftheemployeefirst.C.WhyareAccidentsInvestigated?AllthatisneededtorememberthepurposeofaccidentinvestigationisPREVENTION.Yourpurposeinconductinganaccidentinvestigationisnottopinblameonsomeortosatisfythesupervisor,butsimplytopreventarecurrenceoftheaccident.Ifthisisconstantlykeptinmm,itwillhelptheinvestigationimmeasurably.ConductingtheAccidentInvestigation:Thefirststepinanormalaccidentinvestigationistointerviewemployeesinvolved.Therearecertainproventechniquesforsuccessfulinterviewingofanykind,andwhilemostsupervisorsmaybefamiliarwiththem,theyarecertainlyimportantenoughtorepeat. C)0••AccidentInvestigationPage61.Putemployeesatease.Thebestwayofdoingthisistoremindthemofthepurposeoftheinterviews.Remindthenthatyouareinterestedsolelyinpreventingarecurrenceoftheaccident,andthatyoucanonlydothiswiththetheirhelp.Convincethenthattheinterviewsaresimplyajointefforttopreventotheraccidentsthatcouldbemoreserious.Afriendlyandunderstandingmannerisanecessityingainingcooperation.2.conductinterviewsatthesceneoftheaccident,wheneverpossible.Itwillhelptheemployeestoexplainandyoutounderstand.Makeinterviewsasprivateasyoucan.Thiswillnotonlyputemployeesmoreatease,butwillpreventtheirobservationsandideasfrombeinginfluencedbytheopinionsofothers.Italsoavoidspossibleembarrassmentoveranymistakes.3.Askforemployees’versionoftheaccident.Besuretheyunderstandyouwanttheirversion,justastheysawit,andnotdressedupforyourbenefit;thenletthemtellit!Donotinterruptthentoclarify(“areyousayingthat..interpret(“inotherwords,youmean...“),orevaluate(“wellnow,inmyopinion...“).Ifyoudonotunderstandsomething,waituntiltheyhavefinishedtheirstory;thenask.Aboveall,donotmakejudgmentsorjudgmentalremarks.Thiswillonlyputemployeesonthedefensiveanddefeatthewholepurposeoftheinterviews.4.Askanynecessaryquestions.Thekeywordhereisnecessary.Limityourquestionstofacts,asmuchaspossible,particularlyearlyintheinterview.Findout(1)whathappened,(2)whatwasdone,and(3)howitwasdone.Ifyoumustask“why”typequestions(whywereyoudoing..etc.?),waituntilyouhaveallotherinformation.These“why”questionsarelikelytomakeemployeesdefensive.Askopenquestions,onesthatcannotbeansweredyesorno,wheneverpossible.Theywillgetyounuchmoreinformation.Naturally,allquestionsshouldbeaskedinafriendly,constructivemanner.5.Repeatemployees’storyasyourunderstandit.Thiswilldotwothings.First,itwillassurecorrectunderstandingbyallowingemployeestobesureyouunderstandwhattheymeant;notjustwhattheysaid.Second,itgivesthemachancetohearwhattheysaid.Strangelyenough,mostofusdonotlistentoourselveswhilewearetalking,andthiswillgivetheemployeesachancetocorrectthemselves.Beforeendingtheinterviews,checktheobjectivesandguidepostsonthebackoftheinvestigationformtobesureyouhavecoveredeverything. C0AccidentInvestigationPage8FillingOuttheForm-WhyItIsImportant:Rightorwrong,theevaluationandratingoffirst—linesupervisors’accidentinvestigationwillbebased,inlargepart,ontheaccidentinvestigationreports.Reviewedbyuppermanagement,theyrepresentthesupervisors’workandaretakenasanindicationandmeasureoftheirability.Naturally,theirimportancegoesbeyondthisasarecordandtoolinpreventingaccidents,butkeepinmindthatitalsowillrepresentsupervisorsintheeyesofhighermanagement.Forallthesereasons,itwillpaytoknowexactlywhateachquestionmeans,whyitisneeded,andhowitshouldberecorded.1.Whohadtheaccident?Thereasonforidentifyingthepersonmayseemclearenough,butithasspecialsignificanceforfirst—linesupervisors.Toothersitisameansofidentification,buttosupervisorsitisanemployee;theiremployee.RatherthanDOE,JOHNE.,heisoldJack,withthreekidsincollege,apart—timejob,highbloodpressure,andthemostexperiencedemployeeintheshop.Knowingtheseandotherthingsgivesthelinesupervisorsanumberofpossibleleadsinanalyzinganaccident’scause.Higherlevelsofmanagementmaynotbethisfamiliarwiththeemployees,however,soidentifythemfullybygivingfullnames,orifnecessary,socialsecuritynumbers.Inthecaseofpropertydamage,identifythepropertythatsuffereddamage.Bejustasspecificasyouareinidentifyingaperson.Givethenumberifitisamachineorpieceofequipment.2.Whendidithappen?Thismeansthedateandexacttimeoftheaccidentsuchas“9/28/69at3:35P.M.”Thisisneedednotonlytoplacetheaccidentintime,butbecauseitisdirectlyrelatedtoimportantfactorssuchasshift,schedules,andworkload.3.Wheredidithappen?Thisisthelocationoftheaccidentandisneededsootherscantellexactlywheretheaccidenthappened.Thecausesofanaccidentarefrequentlydirectlyrelatedtothelocation.Beasexactasyoucan.
